Health Center to hold Back to School Immunization Day

ANDERSON — Indiana schools require students to meet minimum immunization requirements provided by the Indiana State Department of Health for children 3 years of age through high school.

The Madison County Community Health Centers, Inc. is hosting a Child Health Access Program (CHAP) Immunization Day on July 30 from 9 a.m. to 5 p.m. at its Anderson location, 1547 Ohio Avenue, Anderson.

Immunizations, back-to-school physicals, dental checks and behavioral health assessments will be offered. There will also be book bags, school supplies and other goodies.

Parents will need to bring ID or guardianship papers, shot records, school exam forms, if applicable, and Medicaid or insurance cards, if applicable. Hoosier Healthwise and Healthy Indiana Plan (HIP) applications will also be available.

No appointment is necessary. Free book bags and other items will be given to all children who attend.
